These Terms of Service (“Terms”) govern your use of the website (inachicago.com), our booking app, and the services and facilities of INA — Relax · Regenerate · Rebalance (“INA,” “we,” “us,” “our”), operated by INANNA LLC, located at 308 W. Erie, Suite 100, Chicago, IL 60654. By creating an account, booking, purchasing, or using our services, you agree to these Terms.
1. Eligibility
You must be at least 18 years old to create an account and enter into these Terms. Certain services have additional age, health, or eligibility requirements and may require a consultation or medical clearance before treatment.
2. Our Services
INA offers wellness, beauty, and medical-related services, which may include yoga, pilates, and sound bath classes; injectable skincare; laser and skin treatments; hydrafacials, chemical peels, PRP, and microneedling; regenerative and naturopathic medicine; IV, NAD, and vitamin therapy; chiropractic, acupuncture, and cupping; massage and lymphatic drainage; body contouring; brow, lash, and related beauty services; and retail products and gift cards. Offerings, providers, and pricing may change.
3. Medical and Treatment Disclaimer
Some of our services are medical or health-related and are provided by or under the supervision of licensed professionals. Before certain treatments you will complete intake and informed-consent forms describing the specific risks, benefits, and alternatives.
- Individual results vary. We make no guarantee that any treatment will produce a particular result.
- Not a substitute for your own medical care. Our services do not replace advice from your primary physician. Consult your physician before beginning any new treatment, especially if you are pregnant, nursing, taking medication, or have a medical condition.
- Consultations. Some treatments require a consultation and clearance, and providers may decline a service they determine is not appropriate for you.
Any separate consent, waiver, or release form you sign for a specific service governs that service and is incorporated into these Terms.
4. Accounts and App
You are responsible for keeping your account credentials secure and for all activity under your account. Booking and account services are provided through MindBody and are also subject to MindBody’s terms.
5. Booking, Cancellation, and No-Shows
Appointments and classes are subject to availability. Late-cancellation and no-show policies (including any fees or forfeited deposits or credits) are disclosed at booking. Please arrive on time; late arrivals may be shortened or rescheduled so we can stay on schedule for all clients.
6. Memberships, Packages, and Billing
Membership and package terms, including price, billing cycle, and included benefits, are presented at the time of purchase.
- Auto-renewal: Recurring memberships renew automatically until cancelled. By purchasing a recurring membership, you authorize us (through our payment processor) to charge your payment method on each billing date.
- Cancellation: You may cancel in accordance with the terms disclosed at purchase. Cancellation stops future renewals and does not retroactively refund the current term unless required by law.
- Refunds: Except where required by law or expressly stated, services, packages, and prepaid credits are non-refundable and may expire as disclosed at purchase.
- Price changes: We may change pricing on a going-forward basis with reasonable notice.
7. Gift Cards
Gift cards are redeemable for eligible services and products, are not redeemable for cash except where required by law, and cannot be replaced if lost or stolen. Additional gift-card terms are disclosed at purchase and are subject to applicable law.
8. Retail Products
Retail products are sold subject to availability and any return policy disclosed at the point of sale. Product descriptions are for general information and are not medical advice.
9. Rules of Conduct
You agree to use our facilities, website, and app respectfully and lawfully, to complete required health disclosures honestly, to follow provider instructions and aftercare guidance, and not to harass others or engage in unsafe behavior. We may suspend or terminate access for conduct that endangers others or violates these Terms.
10. User Content
If you post reviews, comments, or other content to us or our platforms, you grant us a non-exclusive, royalty-free, worldwide license to use, display, and reproduce that content in connection with our business. You are responsible for the content you submit and represent that you have the right to submit it. Clinical or before/after photographs are used only as described in our Privacy Policy and any consent you provide.
11. Intellectual Property
The INA name, logos, and all site and app materials are owned by us or our licensors and are protected by intellectual-property laws. You may not copy, distribute, or create derivative works without our permission.
12. Third-Party Services
Our website and app rely on third-party services (such as MindBody, our payment processor, and app stores). We are not responsible for third-party services, and your use of them is subject to their terms.
13. Disclaimer of Warranties
Except for care provided by licensed professionals under applicable professional standards, our website, app, and services are provided “as is” and “as available” without warranties of any kind, express or implied, to the fullest extent permitted by law, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement.
14. Limitation of Liability
To the fullest extent permitted by law, INA and its owners, providers, staff, and affiliates will not be liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, arising from your use of our services. Our total liability for any claim will not exceed the amount you paid to us in the six months before the claim. Some jurisdictions do not allow certain limitations, and nothing in these Terms limits liability that cannot be limited by law, including liability for professional negligence.
15. Indemnification
You agree to indemnify and hold INA harmless from claims and expenses arising out of your use of our services or your violation of these Terms, except to the extent caused by our own negligence or misconduct.
16. Governing Law and Venue
These Terms are governed by the laws of the State of Illinois, without regard to conflict-of-law rules. You agree that the exclusive venue for any dispute not subject to arbitration will be the state or federal courts located in Cook County, Illinois.

17. Changes to These Terms
We may update these Terms from time to time. The “Last updated” date reflects the current version, and continued use after changes means you accept them.
